Our Approach to Specialist Support Coordination


At MR2V, our approach is person-centred, collaborative, and flexible. From the very start of your service journey, we focus on understanding your experiences, goals, and preferences. We work to amplify your voice because you are in the driver’s seat.


What Does This Mean for You?


  • Not rigid, always adaptable: While we follow a clear pathway and principles, we know life changes. We’ll adjust plans to meet emerging needs, always in line with NDIS guidelines.


  • Your voice matters: We create documentation that helps others understand how to work with and support you effectively. This documentation always reflects your preferences, support needs and what’s important to you.


  • Collaboration is key: We work with you, your care team, and professionals from other service systems, to make sure supports are connected and effective.


  • Reducing risk through communication: Clear communication and planning to help keep you safe and ensure your preferences are respected. We lead care teams through complexity and dysfunction with a collaborative approach, grounded in evidence-based practice. Every decision and strategy explicitly upholds human rights principles autonomy, dignity, and informed choice so that support remains person-centred and least restrictive.

What is Specialist Support Coordination (SSC)?


Specialist Support Coordination is designed for participants with complex needs or high-risk situations. At MR2V, our SSC team brings advanced skills to help stabilise supports and respond to challenges quickly and effectively.


What Makes SSC Different?


Our role is different from Level 2 Support Coordination. We:


  • Respond to and we will work with you and your team when you are experiencing crisis situations and develop harm minimisation plans and develop harm minimisation plans. Harm minimisation plans outline strategies to reduce the risks associated with substance use while respecting individual rights and supporting informed choices.


  • Work closely with care teams, guardians, and mainstream services.


  • Complete comprehensive reports and risk assessments.


  • Support participants with justice involvement, mental health needs, homelessness or housing exclusion, and other complex circumstances.

However, our approach is person-centred, collaborative, and adaptable. At any point in the plan period, we can submit new evidence and report to the agency to ensure your plan is always reflective of your emerging needs.



What Happens When Supports or High-Risk Situations Stabilise?


As Specialist Support Coordination is a time‑limited service, we will work with you and your team to stabilise your situation, strengthen your circle of supports, and, when the crisis has resolved and the NDIS no longer requires Specialist Support Coordination, celebrate this achievement together and transition out smoothly with a strong team that understands your needs, preferences, and goals moving forward .

What a Transition Could Look Like:


We work with many organisations that provide Level 2 Support Coordination. If your funding changes and Level 3 Specialist Support Coordination is no longer required or funded, we can support you through this transition by:


If you are already linked with a Level 2 Support Coordinator:


  • Discussing the change in your plan with you and/or your Plan Nominee.

  • Providing a clear timeframe for the handover.

  • Completing a thorough and robust handover to your Level 2 Support Coordinator.

  • Providing you and/or your Plan Nominee with a copy of the handover documentation.

  • Celebrating the progress and successes achieved during Specialist Support Coordination.


If you are not currently linked with a Level 2 Support Coordinator:


  • Discussing the change in your plan with you and/or your Plan Nominee.

  • Providing linkages to ensure a smooth and supported transition.

  • Completing a referral to a Level 2 Support Coordination service.

  • Providing a clear timeframe for the handover.

  • Completing a thorough and robust handover to the new service.

  • Providing you and/or your Plan Nominee with a copy of the handover documentation.

  • Celebrating the successes and positive outcomes achieved.


Frequently Asked Questions with Specialist Support Coordination


1. What reports does my Specialist Support Coordinator (SSC) write?


Your SSC can prepare:


  • Monitoring Reports – to track progress and outcomes.

  • End of Plan (EOP) Reports – these are a requirement from NDIS and help with future planning.


2. Does MR2V close over the Christmas period?


We observe public holidays and have a two-week Christmas closure.

Don’t worry—we’ll work with you before the break to make proactive plans, so your supports continue smoothly


3. What happens if my SSC goes on leave?


If your SSC is away for more than three days, we’ll put a handover plan in place, so you know who to contact and your supports remain consistent.


4. What happens if I don’t have Level 3 funding in my plan?


As a specialist service, we’ll support you to transition to an external provider. This includes:


  • A robust handover

  • Linkages to ensure a smooth transition of services.


5. Do you have a process around transitions?


Yes! We follow a structured transition process that prioritises:


  • Clear communication

  • Participant involvement

  • Risk minimisation
