Persistence Leads to Life-Changing Support: Support Coordination

When I first received our participants case at My Right 2 Voice, they had already been through three unsuccessful attempts at a change of circumstances request to get the additional support they needed.

Understanding the Challenge:

Our participant was living in their own home, but it needed extensive modifications to create a safe and suitable environment. They also needed significant help with daily activities. Without the right level of support, their ability to live safely and comfortably at home was at risk.

I was determined to help, so I took a strategic and collaborative approach. I worked closely with participants Guardian, Occupational Therapist (OT), and Behaviour Support Practitioner to understand what had been tried before. I also took time to learn more about the participants diagnosis so I could speak about their needs and goals effectively and confidently.

Building a Strong Evidence Base & Overcoming Obstacles:

I carefully reviewed the previous NDIS outcome letters and made sure every part of my new change of circumstances request had strong supporting evidence. I referenced legislative frameworks to back up the request, building a compelling case for why the participant qualified for increased support.

The process definitely had its challenges. Getting funding for Specialist Disability Accommodation (SDA) and Supported Independent Living (SIL) required persistence, creative problem-solving, and an unwavering commitment to the participant best interests.

Understanding the complexities of the home environment, the OT and I took an innovative approach by leveraging the OT's background as a builder. Together, we provided a detailed breakdown of necessary home modifications with cost estimates, helping the NDIS understand the value-for-money aspects of our request.

Life-Changing Results:

Our efforts paid off. When the new NDIS plan was approved, our participant received a significant increase in funding, exactly what they needed to live safely. This outcome had a profound impact, allowing them to maintain their independence while getting the necessary assistance to thrive in their own home.

Looking back on this journey, I found it both challenging and deeply rewarding. I learned that no matter how many times you try, you have to keep pushing.

It was incredibly joyful to hear that our evidence was strong enough to support the request and to be able to tell the care team that we had secured the funding needed to reduce risks for the client.

Celebrating Success::

This success will guide my approach with clients facing similar challenges in the future. In Support Coordination, by using outcome letters as a framework for my action plan and ensuring a thorough, evidence-based strategy, I've established a benchmark for persistence and innovation in securing client support.

Future Impact:

The participants story demonstrates the impact that our My Right 2 Voice team can have in supporting them to transform their lives. Through collaboration, expertise, and dedication, we continue to make meaningful, lasting changes in the lives of those we support.
